Please review the University Survey Procedures prior to submitting the Survey Request Form. Please submit the following survey request form for ALL survey needs.  Following the receipt of the request, the Director of Research and Analytics will contact the requestor to confirm the details of the survey request. Requests will be taken in the order in which they are received. The timeframe for completion will be prioritized based upon the purpose of the survey and the complexity. Please allow 4 weeks when planning for your survey.

Data requests should be received at least four weeks prior to the date the data is needed. Please keep in mind that complex requests may take more time. Following the receipt of the request, the Director of Research and Analytics will contact the requestor to confirm the details of the data request.
